Direct Primary Care “Concierge” services include direct access to primary care by your provider 24/7 via secure text, call video, and/or in person, even on evenings and weekends, unlimited in office and online appointments, longer appointment times, wellness visits with emphasis on disease prevention and health promotion, annual physicals, acute and chronic disease management, comprehensive hormone evaluation and treatment, chronic weight management, breast feeding and lactation support, specialized mental health care, integrative treatment, including lifestyle modifications, medication, and/or therapy when appropriate, in-office Fit3d body scan and infinity massage chair, arrangement of low cost medications and screenings, and referrals/collaboration of care with specialists when needed

Perinatal physical and mental health refers to exacerbation or new onset of physical and mental health disorders during a period of time surrounding childbearing age, preconception, pregnancy, postpartum, post loss, the baby’s first year, and beyond at times. Reproductive and integrative physical and mental health refers to evaluation across the reproductive lifespan from premenstrual symptoms, through challenging fertility journeys, pregnancy, postpartum, and into perimenopause. Management involves looking at biological, psychological, and lifestyle causes and promoting physical, mental, emotional, spiritual, social, and environmental wellness.

Payment options include affordable monthly or annual membership fees with no additional fees or copays per visit. Insurance is not required to become a member of our practice. The Direct Primary Care Membership is not an insurance plan and it is recommended to be paired with an affordable cost sharing or high-deductible plan to ensure you are covered in the event you need a high-cost hospital stay, specialist visit, or expensive testing or procedure. Our goal with increased access and personalized women’s care is to improve quality of care, avoid costly urgent care and emergency room visits, limit expensive testing, lower overall cost, and improve health.

The first appointment can be in office or online “tele-health” and will include a detailed history, screenings, evaluation, assessment, and additional work up, such as labs or imaging, when appropriate followed by a personalized treatment plan including the entire support system and care team. 

Another covering and collaborating provider (nurse practitioner and/or doctor) will provide care if the provider is not available. We recommend seeking care at a local emergency room in crisis and emergency situations and will follow closely upon discharge.

Close follow up, ongoing revaluation, support, management, and personalized care developed by the patient and provider together while building a trusting relationship. We provide full service care and can also refer to specialists when needed and will collaborate with them closely.
